PROVIDENCE ELDERPLACE

Imagine a program that provides older adults with all their medical care, medications, and equipment and combines this with a network of support from social workers, therapists, and in-home caregivers. This program exists and it’s called PACE, a Program of All-Inclusive Care for the Elderly. PACE is both an insurance and a provider. Care can be at a Providence ElderPlace PACE health and social center, or at home when needed.

CLACKAMAS COUNTY AGING & DISABILITY RESOURCE CONNECTION

The Clackamas ADRC is designed to streamline access to information and resources about long-term supports and services in the community. ADRC has information about in-home care, long-term care planning,
how to arrange for home-delivered meals,

how to apply for Medicaid, and much more.

Concerned about an older loved one? Looking for long-term care? Interested in senior centers? Call the Aging and Disability Resource Connection (ADRC) at 503-988-3646 or email adrc@multco.us(link sends e-mail), for 24-hour information and assistance to seniors (older people),

people with disabilities, and caregivers.

Get needs assessed, identify services and connect with providers of those services.
